Registration – Lifetime Member

Note:

1. Field with “*” is required and must be filled in before submitting the form.
2. For user name: letter, number and underscore is allowed. Space is NOT allowed in user name.
3. Your membership application will be pending after submitting the registration form.
4. CAPA admin will review your registration. You will receive email notification once it’s approved.
5. If you register with regular or lifetime level, please remember to pay the membership fee by logging to CAPA website and making a payment. CAPA website will change your membership status to “active” once payment processed.
